You're sending emails. They're going out. But you're getting 2-3% reply rates instead of 8-12%, or your domain reputation is tanking, or you keep hitting sending limits right when campaigns start working.
The problem isn't your copy. It's not your list. It's your infrastructure - and most people don't realize how broken it is until they've wasted months and thousands of dollars trying to scale something fundamentally fragile.
Here's what broken infrastructure looks like, and how to actually fix it.
You're Sending From the Wrong Place
This is the biggest one. You're using Gmail or Google Workspace or Office 365 directly - maybe through Lemlist or another platform - and wondering why things fall apart at scale.
The issue: these providers weren't built for sending cold email. Google Workspace has a 500-email-per-day limit. Microsoft 365 has reputation algorithms that trigger after 1,000 sends. Gmail throttles you if you exceed normal user behavior. And the second you hit these limits or your account gets flagged, your entire operation stops.
The fix: use a dedicated email sending infrastructure. This means:
- A dedicated SMTP provider (Mailgun, SendGrid, AWS SES) - not Gmail
- Your own domain with proper SPF, DKIM, and DMARC records
- Warm IPs (not shared IPs from consumer email providers)
- Multiple sending accounts across different providers so you're not dependent on one

The most practical version: start with one primary domain. Use it for only high-quality prospects (no purchased lists). Have a second domain ready as a backup. If your primary domain gets blacklisted, switch to the secondary while you rebuild the first one's reputation (takes 6-8 weeks).
You're Not Actually Warming Up
You downloaded an email warmup tool. You set it to auto-warm your mailbox. You assumed it was working.
Most warmup tools do almost nothing. They send emails to other warmup accounts, which immediately mark them as read or archive them. This creates artificial engagement signals, but ISPs see right through it - they know what warmup traffic looks like.
Real warmup involves:
- Sending to actual prospects (not warmup services) in small batches initially - 10-20 per day for the first week
- Ramping volume gradually - week 1 is 50 total, week 2 is 150, week 3 is 400, week 4 is 1,000+
- Spacing sends throughout the day (not blasting all 100 at 9am)
- Getting actual replies and engaging with them (real human behavior)
- Letting the account sit for 3-5 days before starting cold campaigns
This takes two weeks minimum. Most people try to compress it into 3-4 days and wonder why their delivery tanks.
You Don't Have a Sending Sequence That Actually Works
You're sending one email and hoping. Or you're sending five emails over two weeks with no logic behind the spacing or messaging.
A real sending sequence looks like this:
Subject: Quick question about [Company Name] Hi [First Name], I work with [similar companies] to [specific result - e.g., reduce their CAC by 30% in 90 days]. Would a 20-minute call make sense to see if there's a fit? Best, [Your Name]
Send this on day 1. If no reply by day 5, send a second email:
Subject: RE: Quick question about [Company Name] Hi [First Name], Didn't hear back - might've gotten lost. Wanted to follow up on my previous email. I'm specifically focused on [specific problem they likely have], which affects [their specific metric]. Worth 20 minutes? [Your Name]
If still no reply by day 10, send once more and then let it go. Three touches maximum. The window where someone will engage is small - after that, you're just sending noise.
The infrastructure piece: automate this sequencing with proper rules. If someone replies, remove them from future sends. If someone unsubscribes, mark them. If an email bounces hard, stop immediately. Most people do this manually or with broken automation, which means you're accidentally sending four emails to someone who already replied.
You Don't Have Monitoring
You send a campaign and hope it works. You don't check delivery rates, bounce rates, complaint rates, or engagement trends until weeks later when you realize something is wrong.
Real infrastructure means daily monitoring of:
- Bounce rate (should be under 3%)
- Complaint rate (should be under 0.1%)
- Delivery rate (should be over 95%)
- Open rate trends (if drops below 6% month-over-month, something is wrong)
- Reply rate by domain (Gmail usually 8-12%, corporate domains 10-15%)
- Sending account health (is Google flagging suspicious activity?)
If you see delivery dropping, you catch it on day two, not day twenty. If a domain is getting flagged, you know within hours. If your list quality is degrading, the metrics tell you before it kills your domain reputation.
Most people ignore these numbers entirely and wonder why their entire system collapses suddenly.
The Gap Between Knowing This and Actually Running It
You can read this and understand what needs to happen. But implementing it means setting up SMTP servers, configuring DNS records, building automation rules, monitoring daily metrics, managing multiple accounts, rebuilding domain reputation when things break, and keeping all of this running consistently for months.
That's the gap - between knowing your infrastructure is broken and actually having something that works at scale.
Some businesses build this themselves. Most don't, because it requires technical knowledge they don't have and operational discipline they can't sustain while running everything else.
That's specifically what proper cold email infrastructure setup solves - not just knowing what to do, but having it built, monitored, and maintained by people who do this every day.
